About the Role
As a Physical Therapist Assistant in home health, you’ll work under the supervision of a licensed Physical Therapist (PT) to deliver high-quality, compassionate care. You will assist patients recovering from illness, injury, or surgery—helping them achieve their highest functional potential in a familiar and comfortable setting.
You’ll also collaborate closely with nurses, physicians, and caregivers to ensure patients receive the best possible care at home.
Your Responsibilities
- – Assist in implementing individualized treatment plans as directed by the supervising PT.
- – Deliver therapeutic exercises, manual therapy, and other interventions to promote recovery
- – Monitor patient progress and promptly report changes to the PT, case manager, or physician.
- – Educate patients and caregivers on exercises, safety techniques, and home therapy programs.
- – Document all care activities accurately and in compliance with regulatory standards.
- – Support discharge planning to ensure a smooth transition and continuity of care.
- – Maintain a safe and clean work environment by following infection control and safety guidelines.
- – Uphold a safe and clean working environment by adhering to infection control protocols and safety guidelines.
- – Adhere to established Agency policies and procedures.
- – Participate in interdisciplinary team meetings and patient care conferences.
- – Participate in patient care planning with other healthcare personnel.
- – Consult with the PT regarding any necessary changes in treatment.
- – Attend patient care conferences as scheduled.
Qualifications
- – Graduate of an accredited Physical Therapist Assistant program.
- – Washington State PTA license (or eligibility for licensure).
- – Valid Washington driver’s license and proof of insurability.
- – Current CPR/First Aid certification required.
- – Strong communication and documentation skills.
- – Compassionate, dependable, and self-motivated with a passion for patient-centered care.
- – Experience in home health, acute care, or rehabilitation settings preferred but not required.
- – Strong interpersonal skills with the ability to work collaboratively in a team-oriented environment.
- – Commitment to providing compassionate patient-centered care.
- – Ability to exercise initiative and independent judgment.
Compensation & Benefits
- Pay: $35.00 – $55.00 pay per visit/unit, DEO
