Annexia Home Health is seeking a compassionate and skilled Home Health Physical Therapist in Federal Way, WA to join our dedicated team. In this rewarding role, you’ll provide personalized, one-on-one in-home therapy that helps patients regain mobility, independence, and confidence in the comfort of their own homes, following best-practice guidelines from American Physical Therapy Association (APTA).

With an average caseload of 4-6 patients per day and flexible scheduling, you’ll have the autonomy to build meaningful therapeutic relationships while maintaining work-life balance. About the Role

As a Physical Therapist Assistant in home health, you’ll work under the supervision of a licensed Physical Therapist (PT) to deliver high-quality, compassionate care. You will assist patients recovering from illness, injury, or surgery—helping them achieve their highest functional potential in a familiar and comfortable setting.

You’ll also collaborate closely with nurses, physicians, and caregivers to ensure patients receive the best possible care at home.

Your Responsibilities

  • Assist in implementing individualized treatment plans as directed by the supervising PT.
  • Deliver therapeutic exercises, manual therapy, and other interventions to promote recovery
  • Monitor patient progress and promptly report changes to the PT, case manager, or physician.
  • Educate patients and caregivers on exercises, safety techniques, and home therapy programs.
  • Document all care activities accurately and in compliance with regulatory standards.
  • Support discharge planning to ensure a smooth transition and continuity of care.
  • Maintain a safe and clean work environment by following infection control and safety guidelines.
  • Uphold a safe and clean working environment by adhering to infection control protocols and safety guidelines.
  • Adhere to established Agency policies and procedures.
  • Participate in interdisciplinary team meetings and patient care conferences.
  • Participate in patient care planning with other healthcare personnel.
  • Consult with the PT regarding any necessary changes in treatment.
  • Attend patient care conferences as scheduled.

Qualifications

Required Qualifications:

  • Graduate of an accredited Physical Therapist Assistant program.
  • Washington State PTA license (or eligibility for licensure).
  • Valid Washington driver’s license and proof of insurability.
  • Current CPR/First Aid certification required.
  • Strong communication and documentation skills.
  • Compassionate, dependable, and self-motivated with a passion for patient-centered care.
  • Strong interpersonal skills with the ability to work collaboratively in a team-oriented environment.
  • Commitment to providing compassionate patient-centered care.
  • Ability to exercise initiative and independent judgment.

Preferred Qualifications:

  • Minimum of one year of home health physical therapy assistant (PTA) experience
  • Experience in home health, acute care, or rehabilitation settings
  • Familiarity with electronic medical record (EMR) systems
  • Bilingual skills (Spanish, Russian, Vietnamese, or other languages spoken in our community)

Compensation & Benefits

Compensation:

  • Competitive Pay: $35.00 – $55.00 pay per hour/unit, depending on experience (DEO)
  • Earning Potential: $72,800 – $114,400+ annually
  • Payment Schedule: Bi-weekly direct deposit

Job Types:

  • Part-time or PRN (Per Diem) – Build your own schedule
  • Full-time
