About the Role

As a Physical Therapist Assistant in home health, you’ll work under the supervision of a licensed Physical Therapist (PT) to deliver high-quality, compassionate care. You will assist patients recovering from illness, injury, or surgery—helping them achieve their highest functional potential in a familiar and comfortable setting.

You’ll also collaborate closely with nurses, physicians, and caregivers to ensure patients receive the best possible care at home.

Your Responsibilities

• – Assist in implementing individualized treatment plans as directed by the supervising PT.
• – Deliver therapeutic exercises, manual therapy, and other interventions to promote recovery
• – Monitor patient progress and promptly report changes to the PT, case manager, or physician.
• – Educate patients and caregivers on exercises, safety techniques, and home therapy programs.
• – Document all care activities accurately and in compliance with regulatory standards.
• – Support discharge planning to ensure a smooth transition and continuity of care.
• – Maintain a safe and clean work environment by following infection control and safety guidelines.
• – Uphold a safe and clean working environment by adhering to infection control protocols and safety guidelines.
• – Adhere to established Agency policies and procedures.
• – Participate in interdisciplinary team meetings and patient care conferences.
• – Participate in patient care planning with other healthcare personnel.
• – Consult with the PT regarding any necessary changes in treatment.
• – Attend patient care conferences as scheduled.

Qualifications

• – Graduate of an accredited Physical Therapist Assistant program.
• – Washington State PTA license (or eligibility for licensure).
• – Valid Washington driver’s license and proof of insurability.
• – Current CPR/First Aid certification required.
• – Strong communication and documentation skills.
• – Compassionate, dependable, and self-motivated with a passion for patient-centered care.
• – Experience in home health, acute care, or rehabilitation settings preferred but not required.
• – Strong interpersonal skills with the ability to work collaboratively in a team-oriented environment.
• – Commitment to providing compassionate patient-centered care.
• – Ability to exercise initiative and independent judgment.

Compensation & Benefits

• Pay: $35.00 – $55.00 pay per visit/unit, DEO
• Job Types: Part-time or PRN
