Job Summary
The Occupational Therapist will provide diagnostic evaluations and treatments to assigned patients, helping them reach their maximum performance level and utilize skills to function within the community.
Responsibilities
- Conduct ongoing interdisciplinary assessments and develop individualized plans of care in partnership with patients, representatives (if any), and caregivers.
- Provide services as ordered by the physician and indicated in the plan of care.
- Educate and counsel patients, caregivers, and families.
- Communicate with physicians and other healthcare practitioners regarding the home health plan of care.
- Evaluate patients’ functional status, home environments, and equipment needs, participating in the development of the total plan of care.
- Develop treatment programs and establish goals for improved function.
- Teach new skills or retrain patients in daily activities lost due to illness or injury, including the use of prosthetic and/or orthotic devices.
- Identify needs for other home health services and make appropriate referrals.
- Supervise Certified Occupational Therapy Assistants according to organization policy and state regulations.
- Participate in discharge planning for patients.
- Assess and screen patients’ daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests of function to assist physicians in evaluating patients’ level of function.
- Provide therapeutic treatment and instruction to improve or restore strength, coordination, range of motion, and function or teach compensation measures.
- Consult with vocational team members to develop and implement vocationally oriented plans consistent with patients’ needs and capabilities.
- Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, and family members.
- Participate in care coordination and discharge planning activities, acting as a resource to other healthcare personnel.
- Design community reintegration activities to assist clients in physical reconditioning and psychological adjustment.
- Recommend and design adaptive equipment to improve clients’ living or working environments and participation in rehabilitation programs.
- Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills.
- Supervise and evaluate Occupational Therapy Assistants and Home Health Aide performance in implementing occupational therapy services.
- Document patients’ care observations, interventions, and evaluations accurately and thoroughly on the day services are rendered. Ensure interim (verbal) orders from physicians are documented and implemented. Submit evaluations, treatment plans, and discharge summaries to supervisors and care management staff.
Requirements
- Degree from an accredited Occupational Therapy Program.
- Current Washington State licensure to practice as an Occupational Therapist.
- Minimum of 1 year of Occupational Therapy experience preferred.
- Home health experience is preferred.
- Demonstrates good verbal and written communication and organizational skills.
- Possesses and maintains current CPR Certification.
- Valid driver’s license, auto insurance, and reliable transportation required.
