Supporting Patients of All Ages at Home
Annexia Home Health serves patients of all ages who need skilled care at home after hospitalization, surgery, or a change in health status, as well as those living with chronic conditions such as heart failure, COPD, stroke, and diabetes. Patients are often referred by discharge planners, physicians, and rehabilitation facilities when home health is the safest next step in their recovery.
Typical patients
We commonly serve patients who are:
- Recently discharged from the hospital, skilled nursing facility, or rehab.
- Managing complex or chronic conditions that require close monitoring at home.
- Experiencing mobility limitations, balance issues, or high fall risk.
- Needing support to manage medications, wounds, or a new diagnosis.
How home health care works
1. Referral and eligibility
A physician or hospital team sends a referral, and we confirm eligibility for home health and available insurance coverage.
2. In-home assessment
A skilled clinician visits your home to review your health history, medications, home safety, and personal goals.
3. Personalized care plan
Together with your physician, we design a plan of care tailored to your needs, schedule, and home environment.
4. Ongoing visits and coordination
Our nurses and therapists visit regularly, monitor your progress, teach you and your caregivers, and keep your physician informed to help prevent avoidable hospital readmissions.
