Learn How to Get Started with Home Home Care Journey
The first step toward receiving home health care is to obtain a physician's orders/referral and partner with Annexia Home Health to develop a personalized care plan. From there, you will schedule an initial consultation with our team, during which we'll assess your needs and develop a tailored treatment plan. We encourage family members and other caregivers to join the planning process for comprehensive care.
When services begin, our home health care staff will implement the plan – following all physician orders – and keep your doctor updated on your progress. We will adjust the frequency and type of home health visits based on your needs. Some patients require daily care while others require only a short visit once or twice per week.
We tailor all services to meet your specific requirements.
The following topics are commonly discussed during admission.
- Patient's Health Review: We'll review your medical history, current health status, and medication management to fully understand your medical needs.
- Personalized Care Plan: Together, we'll set care goals, outline specific services, and involve your family members to support your care.
- Home Safety & Equipment: We'll assess your home for safety and identify any necessary medical equipment to ensure a safe environment.
- Logistics of Schedule: We'll develop a detailed schedule for our visits, set the frequency and type of home health visits, and ensure coordination with all parties involved in your care.
- Emergency Preparedness: We'll develop an emergency plan, including contact information for healthcare providers and clear instructions for handling medical emergencies.
