Position Overview:

As a Home Health Aide at Annexia Home Health, you’ll play a crucial role in supporting our patients’ daily living activities and assisting with personal care tasks. You’ll work closely with our nursing staff to ensure the highest quality of care for our clients.

Key Responsibilities:

• Assist patients with personal care activities such as bathing, dressing, and grooming
• Help with mobility, transfers, and positioning
• Perform light housekeeping duties to maintain a clean and safe environment
• Prepare and serve nutritious meals according to dietary requirements
• Monitor and report changes in patient condition to nursing staff
• Provide companionship and emotional support to patients
• Assist with medication reminders (as permitted by state regulations)
• Accurately document care provided and maintain patient records

Qualifications:

• Current CNA certification
• High school diploma or equivalent
• Minimum of 6 months experience as a CNA or Home Health Aide preferred
• Valid driver’s license and reliable transportation
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and patient-centered approach to care
• Physical ability to assist with patient transfers and mobility
