Home Health Physical Therapist (PT) – Part-Time

Are you passionate about helping patients regain their independence and improve their quality of life? Annexia Home Health is seeking a skilled and compassionate Part-Time Home Health Physical Therapist to join our dedicated team.

Your Role as a Home Health Physical Therapist:

• Plan and administer personalized physical therapy treatments for patients with various injuries, illnesses, and functional disabilities
• Conduct comprehensive evaluations to assess patients’ physical strengths and functional levels
• Develop and implement conditioning and rehabilitation programs aligned with physicians’ treatment plans and patient goals
• Collaborate with our interdisciplinary team to provide holistic, patient-centered care
• Provide guidance and supervision to Physical Therapy Assistants and Home Health Aides
• Maintain accurate and timely documentation of patient care, progress, and outcomes

Qualifications:

• Degree from an accredited Physical Therapy Program (APTA-approved)
• Current and unrestricted Physical Therapy license
• Minimum of one year of physical therapy experience preferred
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation
