Annexia Home Health is seeking a compassionate and skilled Home Health Physical Therapist in Federal Way, WA to join our dedicated team. In this rewarding role, you’ll provide personalized, one-on-one in-home therapy that helps patients regain mobility, independence, and confidence in the comfort of their own homes, following best-practice guidelines from American Physical Therapy Association (APTA).

With an average caseload of 4-6 patients per day and flexible scheduling, you’ll have the autonomy to build meaningful therapeutic relationships while maintaining work-life balance. About the Role

As a Physical Therapist (PT) with Annexia Home Health, you’ll evaluate patients, develop personalized treatment plans, and guide them through rehabilitation programs designed to restore function and quality of life. You’ll have the clinical autonomy to make treatment decisions while collaborating closely with our interdisciplinary team—including nurses, occupational therapists, and social workers—to ensure comprehensive, patient-centered care.

You’ll primarily serve patients in Federal Way and surrounding South King County communities, with most patients located within a 15-mile radius. Our patient population includes post-surgical rehabilitation, stroke recovery, orthopedic conditions, balance and fall prevention, and chronic disease management. We use a modern EMR system for documentation, allowing you to complete most charting during or immediately after visits.

Your Responsibilities

  • Plan and administer personalized physical therapy treatments for patients with various injuries, illnesses, and functional disabilities
  • Conduct comprehensive evaluations to assess patients’ physical strengths, limitations, and functional levels
  • Develop and implement evidence-based conditioning and rehabilitation programs aligned with physicians’ treatment plans and patient goals
  • Collaborate with our interdisciplinary team to provide holistic, patient-centered care and ensure seamless care coordination
  • Provide guidance, mentoring, and supervision to Physical Therapy Assistants and Home Health Aides
  • Maintain accurate and timely documentation of patient care, progress, and outcomes in our electronic medical record system
  • Participate in care conferences and communicate regularly with physicians, case managers, and family members
  • Ensure compliance with all regulatory requirements and agency policies

Qualifications

Required Qualifications:

  • Graduate of an accredited Physical Therapy program (DPT or MPT)
  • Current, unrestricted Washington State Physical Therapist license
  • Valid Washington State driver’s license and proof of auto insurance
  • Current CPR/BLS certification
  • Excellent communication, organization, and documentation skills
  • Strong clinical assessment and critical thinking abilities
  • Passion for patient-centered care and improving lives through movement

Preferred Qualifications

  • Minimum of one year of home health physical therapy experience
  • Experience with geriatric populations and common home health diagnoses
  • Familiarity with electronic medical record (EMR) systems
  • Bilingual skills (Spanish, Russian, Vietnamese, or other languages spoken in our community)

Compensation & Benefits

Compensation:

  • Competitive Pay: $55.00 – $75.00 per hour/unit, depending on experience (DEO)
  • Earning Potential: $114,400 – $156,000+ annually
  • Payment Schedule: Bi-weekly direct deposit

Typical Day:

  • Start your day with flexibility—no mandatory office hours or morning meetings
  • See 4-6 patients within a focused geographic area (most within 15 minutes of each other)
  • Spend 45-60 minutes with each patient, allowing time for thorough assessments and meaningful interventions
  • Complete documentation using our mobile/tablet-friendly EMR system during or immediately after visits
  • Average 1-4 new evaluations per week, with the rest as follow-up visits
