Physical Therapist (PT)

Annexia Home Health is looking for a compassionate and skilled Home Health Physical Therapist in Federal Way, WA to join our dedicated team. In this rewarding role, you’ll provide high-quality, one-on-one in-home therapy to help patients regain mobility, independence, and confidence—all in the comfort of their own homes.

About the Role

As a Physical Therapist (PT) with Annexia Home Health, you’ll evaluate patients, develop personalized treatment plans, and guide them through rehabilitation programs designed to restore function and quality of life. You’ll collaborate closely with our interdisciplinary team, ensuring continuity of care and positive patient outcomes.

Your Responsibilities

• – Plan and administer personalized physical therapy treatments for patients with various injuries, illnesses, and functional disabilities
• – Conduct comprehensive evaluations to assess patients’ physical strengths and functional levels
• – Develop and implement conditioning and rehabilitation programs aligned with physicians’ treatment plans and patient goals
• – Collaborate with our interdisciplinary team to provide holistic, patient-centered care
• – Provide guidance and supervision to Physical Therapy Assistants and Home Health Aides
• – Maintain accurate and timely documentation of patient care, progress, and outcomes

Qualifications:

• – Graduate of an accredited Physical Therapy program.
• – Current Washington State PT license.
• – Valid Washington State driver’s license and proof of auto insurance.
• – Minimum of one year of home health physical therapy experience preferred
• – Current CPR/BLS certification
• – Excellent communication, organization, and documentation skills.
• – A passion for patient-centered care and improving lives through movement.

Compensation & Benefits

• Competitive Pay: $55.00 – $75.00 pay per visit/unit, DEO
• Job Types: Part-time or PRN
