Medical Social Worker (MSW) Federal Way – Annexia Home Health is hiring a PRN / Part-Time MSW to provide professional medical social services to patients in their homes as ordered by a physician. This role supports holistic patient care by addressing emotional, social, environmental, and community resource needs. Additionally, it ensures best-practice social work standards are consistently upheld throughout the entire care continuum.
About the Role
A Medical Social Worker (MSW) provides social work services to patients on an intermittent basis in their place of residence. Services are delivered in accordance with physician orders and the established plan of care under the direction and supervision of the Director of Clinical Services or Clinical Manager.
Services are furnished by a qualified social worker or by a qualified social work assistant under the supervision of a qualified social worker, consistent with Washington State regulations and federal home health Conditions of Participation.
The MSW conducts comprehensive psychosocial assessments, develops individualized care plans, coordinates community resources, and provides short-term counseling and crisis intervention. In addition, the MSW provides required clinical supervision to Bachelors-prepared Social Workers (BSWs) in accordance with regulatory standards.
Key Responsibilities
- Perform comprehensive initial and ongoing psychosocial assessments for each assigned patient.
- Develop, implement, and evaluate social work plans of care collaboratively with patients, RN Case Managers, and physicians.
- Assess psychosocial status, caregiver capacity, environmental safety, and community resource needs.
- Provide short-term crisis intervention, counseling, and facilitation of support systems consistent with the plan of care.
- Evaluate environmental safety risks and make appropriate recommendations.
- Assist patients and representatives with accessing community and financial resources (housing, utilities, transportation, medication assistance, legal support, advance directives, etc.).
- Identify psychosocial risk factors including potential suicide, abuse, or neglect and complete required reporting.
- Identify barriers to compliance and support understanding of treatment goals.
- Maintain timely, accurate clinical documentation in accordance with agency policies and regulatory standards.
- Participate in Quality Assessment and Performance Improvement (QAPI) activities.
- Provide required monthly supervision to BSW staff or social work assistants (if applicable).
- Promote patient, personnel, and workplace safety while adhering to universal precautions.
- Meet established PRN productivity standards.
Qualifications
Required Qualifications:
- Master of Social Work (MSW) degree accredited by the Council on Social Work Education (CSWE).
- Minimum one (1) year of social work experience in a healthcare setting.
- Current Washington State Social Worker license.
- Clean criminal background check.
- Current professional liability and malpractice insurance.
- Valid Washington State driver’s license with reliable, insured transportation.
- Demonstrated ability to practice independently and make sound clinical judgments.
- Strong written and verbal communication skills.
- Computer literacy and proficiency with electronic documentation systems.
- Physical ability to perform essential job functions, including mobility required for home visits.
Preferred Qualifications:
- Minimum of one (1) year of home health or hospice social work experience.
- Experience working with Medicare and Medicaid populations.
- Knowledge of community resources in King and Pierce Counties.
- Experience conducting suicide risk, abuse, or neglect assessments.
- Experience supervising BSW staff or social work assistants.
- Bilingual abilities (Spanish or other languages prevalent in the region).

Compensation:
- Competitive Pay: $40.00 – $45.00 per hour/unit, depending on experience (DEO)
- Earning Potential: $83,200 – $93,600+ annually
- Payment Schedule: Bi-weekly direct deposit
